Methods of Payment:
We accept cash, check, money orders, Visa, Master Card and American Express for treatment and supplements.
Insurance Accepted:
Most insurance accepted. We are contracted with most Medicare and Blue Shield PPO plans. We do not accept American Specialty Health (ASH), Covered California, Medi-Cal or Kaiser. We are happy to bill any insurance, however the patient is primarily responsible for understanding their plan.
Disclosure: Please note that insurance coverage is not a guarantee of payment and chiropractic coverage depends on your specific plan.
Patient Forms:
We want to make your appointments with us more convenient for you. We are providing our Case History, Pain Chart, and HIPAA Notice of Privacy Practices forms for you to print, fill out in the privacy of your home, and either bring to the office on your first visit or FAX to us at (530) 273-6826.

Cancellation Notice:
If you are unable to keep your appointment, please notify us as soon as possible and no later than 24 hours ahead of time. Appointments canceled after the 24 hours prior to appointment time are subject to a $25 charge.
